If the participant is under any medication or has any allergies(eg: food, dust or drug) ; please provide such details to us. Any specific characteristics like night walking, phobias(eg: height or water) etc. should be mentioned in the space below. This will help us take care of the participant during the camp.

Declaration

I would like to enroll myself / my ward to participate in the nature / adventure camp organized by you. I am aware of the risks inherent in the camp involving adventure sports / activities during the programme as well as travelling organized and conducted by OUTDOORS UNLIMITED, and their agents. am aware of the risks inherent in tours involving adventure sports / activities such as trekking, rock climbing, rappelling, Tyrolean traverse, vertical ladder, camping, white water rafting, bridge slithering, kayaking, wildlife safaris and any other activities that are organized and conducted by OUTDOORS UNLIMITED and their agents.

I hereby state that I / my ward is participating in these adventure sports / activities of my own free will and consent, and understand and agree that OUTDOORS UNLIMITED, and their agents take all necessary precautions to minimize the risk and avoid any accidents and ensure safe participation in these adventure sports / activities and are not responsible for any mishaps that may occur while participating.

I have read the above statement and the terms and fully understand its contents. I realize and agree that the same serves as an agreement of release and assumption of risk on myself.

I declare that the terms and conditions mentioned overleaf have been explained & interpreted to me in my vernacular language and agree to abide by them. I realize and agree that the same serves as an agreement of release and assumption of risk on myself.

Cheque/ Demand draft should be drawn in favour of Outdoors Unlimited.
Outstation cheques shall not be accepted.
Payment against dishonoured cheque/s shall have to be paid in cash with additional charge of Rs. 250/- per cheque.
The balance payment is to be paid atleast 30 days before the departure of the batch. If the balance payment is not made on time, it would automatically be considered cancelled without any prior intimation and no refund would be given in this regard.
Camp/Batch may be cancelled if there is not enough strength, actual amount paid by the participant towards the programme will be refunded by A/c Payee cheque only. No other claims will be entertained.
No refund shall be given if the participant terminates the camps at any point due to any reason.
Outdoors Unlimited reserves the unconditional right to refuse a booking/terminate a participant on a programme in event of unreasonable behavior at any time and no refund will be given in this regard.
The amount once paid is not transferable to any other programme or to any other person.
Jain food may be served on specific request, subject to availability.
In case the camp duration is extended due to unforeseen circumstances, each participant will have to pay Rs 400/- per head, per day to the group leader.
It shall be the participants responsibility to collect the necessary travel documents from our office on time.
The organisers or any individual connected with the camp shall not be held responsible in case of any accident or any unforeseen natural calamity.
In case of injury or severe illness where upon the participant has to be admitted to the hospital or sent back by flight or any other means of transport. Then those charges shall be entirely borne by the parents and the expense has to be deposited at the office immediately.
Organisers will not be responsible towards any internal disputes arising with fellow participants.
We are not bound to provide name, address or any other personal details of other participants.
The organisers shall not be held responsible for loss/theft of the participants luggage and belongings.
Outdoors Unlimited has the right to use the participants name, information and photographs in promotion materials, brochures and press releases.
The participant may have to pay fine, wherever applicable in the absence of a valid Photo ID.
